Membership Benefits

There are two membership levels for individuals joining the Coalition: ACTIVE and ASSOCIATE.

ACTIVE MEMBERSHIP:
An individual that is the voting member of a practice (1 per practice).

Active members benefit from an association designed to help individual oncology professionals learn from their peers and share ideas, advice, and experiences. Active members are eligible to attend CHOP business and educational meetings (1 regional and 1 state annually) and are eligble to run for board positions.
Annual Active Membership Fee: $50

ASSOCIATE MEMBERSHIP:
Individuals working within an Oncology Hematology Clinic but the Clinic is already represented by an Active Member.

Associate members benefit from an association designed to help individual oncology professionals learn from their peers and share ideas, advice, and experience. Associate members are eligible to attend CHOP business and educational meetings (1 regional and 1 state annually).
Annual Associate Membership Fee: $25

Corporate Membership:

Corporate Member benefits include networking and education opportunities; participation in CHOP events, unique membership opportunities dependant on level of membership; fixed ads on CHOP website and much more.

Please print the membership application, provide the information as requested, and list each individual representative who will be a member and included in the membership directory and mailing list. Send your completed application, along with a check made payable to CHOP to:

After your application is processed and payment is received, you will receive an email confirmation.

Please Note: All CHOP membership runs on the fiscal year of January 1st - December 31st. All membership fees are subject to change and due when the application is submitted, and non-refundable, if application is approved. Individual memberships are non-transferable.
